Upon finishing the survey, you’ll be directed to a confirmation page indicating that your visit has concluded. If you experienced any technical difficulties during the session, you’ll have the opportunity to report those issues directly on that page.

Our medical record software uses your email address as a way of verifying your identity and can be used to help reset your patient portal password if you forget it. We do not sell or share your email address.
